The Faculty of Medicine at the Université Mouloud MAMMERI in Tizi-Ouzou is launching a tender for the purchase of Laboratory and teaching supplies and consumables within the operating budget 2018 Chapter 22-13, Articles 03. The specifications are structured in two (02) lots, namely : Lot no. 1: Supplies and Consumables for laboratories and teaching workshops (Acquisition of Pharmacy Supplies and Consumables)Lot no. 2: Supplies and Consumables for laboratories and teaching workshops (Acquisition of Dental Supplies and Consumables) The present consultation will be publicly posted and published on the website of the University "Mouloud MAMMERI" of Tizi-Ouzou: www.ummto.dz. Tenderers interested in this consultation are invited to obtain the specifications from the Service des Moyens Généraux of the Faculty of Medicine. They may bid for one or more lots.Tenders must include a declaration of candidacy, a technical offer and a financial offer. Each offer is inserted in a closed and sealed envelope, indicating the reference and subject of the consultation as well as the mention "technical" or "financial", as the case may be. The three envelopes are placed in another anonymous envelope, bearing the words: "to be opened only by the bid opening and evaluation committee". and addressed to :Dean of the Faculty of Medicine.

A/ Application form :- A declaration of candidacy ;
- A declaration of probity;
- Copy of articles of association for companies;
- Copy of trade register ;
- Copy of tax identification ;
- An extract from the audited roll or with a schedule dated less than 1 month before the date of submission;
- Original criminal record less than three months old at the date of submission;
- Bank account number and address ;
- Financial statements for the last three years, accompanied by profit and loss statements (TCR) ;
- List of specifications;
- Declaration to be completed, signed, dated and sealed ;
- Up-to-date certificate issued by CASNOS and CNAS (originals) ;
- Certificates of satisfactory performance for the last three years;
- Letter of tender duly completed, signed, dated and sealed ;
- Detailed estimate and quantity signed, dated and sealed ;
- Unit price schedule signed, dated and sealed.
