- Declaration of candidacy duly completed, dated, signed and sealed.
- Declaration of probity duly completed, dated, signed and sealed
- Copy of company statutes.
- Professional reference (certificate of good performance in the field of the service only)
- Financial statements for the last three (03) years, accompanied by RCRs.
- Declaration to be completed, dated and sealed by the bidder
- Listed and initialed specifications with "read and approved" written on the last page.
- Copy of tax identification.
- Bank account number and address.
- Extract from the tax roll, audited or with schedule, dated less than one month before the date of submission.
- Copy of criminal record less than 03 months old at the time of submission.
- Unit price schedule dated, signed and sealed by the bidder
- Estimated and quantitative details dated, signed and sealed by the bidder
- Letter of Tender duly completed, dated, signed and sealed by the bidder
