Social welfare: A practical guide to COS benefits

I - MEDICAL AGREEMENTS (UMMTO - CLINICS)

UMM TO's social services have agreements with clinics for operations, medical analyses, X-rays and other services.

The following are eligible:
- the policyholder,
- the spouse who is not a member of CNAS or CASNOS,
- minor children
- dependent ascendants as defined by CNAS.

30% of the sum is paid by the employee, 70% by the social welfare organization (up to a maximum of 100,000.00 da, with the exception of ophthalmology and dental care up to a maximum of 50,000.00 da).

The fees charged by the various clinics are not the same, so please enquire beforehand.

File to be provided:
 Prescription for the procedure, tests or x-rays to be carried out
 A family sheet in the case of the spouse, child or ascendant.
 A photo of the patient
 A copy of attestation of function
 Attestation of non-salaried activity in the case of the spouse
 Decree of coverage issued by CNAS, in the case of ascendants

II- MEDICAL ENDORSEMENT

Please note: Only medical riders not covered by the agreement will be considered for possible reimbursement.

File to provide :
- Medical prescription (original)
- Report of the procedure performed (intervention, analysis or X-ray)
- Family record form if the insured's spouse or child
- Attestation of non-employment if the spouse does not work
- Statutory invoice (vouchers are not valid)
- Attestation of CNAS coverage if a dependent ascendant
- Pay slip
- Attestation of employment (Copy)

III - AID TO CONCEREUX 

 Dossier à fournir :
- Demande de l'intéressé,
- La prescription médicale (Originale)
- Rapport médical, scanner, résultats anapathe et tout autre document utile.
- Une facture réglementaire (originale et une copie) justifiant le protocole médical.
- fiche de paie (validité de 03 mois)
- -attestation de fonction validité de 03 mois (Copie)
- Une photo du concerné

No medical amendment will be taken into account after a statutory period of ten (10) months.

IV- SOCIAL LOANS

 Dossier à fournir :
- Une demande motivée adressée au président de la COS,
- Une fiche de paie récente (validité 03 mois)
- Attestation de fonction (validité 03 mois)
- Une fois le prêt accordé, fournir l'engagement en trois exemplaires légalisés et une fiche de paie (pour le matricule).

N.B.: - Priority is given to civil servants (teachers and ATS) who have never benefited from a loan. For those who have already benefited, the granting of a loan depends on the assessment of the reasons given.
- Only one loan is granted during the term of office (03 years), except in cases of force majeure.)

V- GIFT VOUCHERS

 An amount of 10,000 DA is transferred to the account of each active employee (ENS and ATS).
 A 9,000 DA voucher will be exchanged for goods retained by COS. Retirees and the following employees are eligible:
* on secondment
* newly recruited on 31/12/21
* employees reinstated in their posts
* transferred (further information will be requested from employees transferred to another establishment).
* contract employees (CDI)
* long-term sickness

NB: In the event of the pensioner's death, the voucher will be given exclusively to:
- The spouse and/or daughter who are not affiliated (CNAS and CASNOS)
- Minor children and disabled children as defined by CNAS.
- His/her unmarried daughter who are not affiliated (CNAS and CASNOS).

VI- RETIREMENT ALLOWANCE 

A donation of around 200,000DA is granted to any ATS civil servant or teacher retiring.
 File to be provided:
- Retirement decision,
- Attestation of cessation of payment
- Crossed cheque in the name of the retiree
- Retirement certificate.

NB// The amount of the retirement indemnity is transferred to the account of the person concerned.

VII - DEATH BENEFIT

A death benefit of 400,000 DDA is granted to the deceased civil servant (Teacher or ATS).
 File to be provided:
- File to be provided by the heirs:
- Death certificate,
- Family file
- Work certificate,
- Original of the fredha
- Notarized power of attorney,
- Copy of the proxy's CNI
- Birth certificate of the proxy
- Crossed cheque of the proxy.

VIII- ASSISTANCE WITH FUNERAL EXPENSES

For the death of an ascendant, descendant or spouse of a teacher or ATS, assistance of around 30,000DA is awarded to the civil servant.

Important note: Only current employees will receive funeral benefits.

IX - WEDDING BONUS

 Documents to be supplied:
- Marriage certificate
- Family record
- Recent pay slip (valid for 03 months)
- Copy of official's certificate of employment (valid for 03 months)

X- CHILDBIRTH ASSISTANCE

 Dossier à fournir :
- Acte de naissance
- Copie d'attestation de fonction du fonctionnaire (validité 06 mois)
- Fiche familiale
- Une fiche de paie récente (validité 03 mois)
NB : No dossier for reimbursement of this allowance will be accepted after 06 (six) months.

XI- AID FOR MULTIPLE CHARITIES, HANDICAPPED PEOPLE, ORPHANAGES

 Dossier à fournir :
- Handicapés :
- Décision ou carte d'Handicap au sens CNAS
- Certificat médical
- Acte de naissance
- Chèque barré ou fiche de paie récente (validité 03 mois)
- Copie d'attestation de fonction du fonctionnaire (validité 06 mois)
- Fiche familiale (lien salarié et l'handicapé)
- Une déchargeée signée du salarié

- Orphanage
- Death certificate of employed parent,
- Family record,
- Barred cheque or recent pay slip (valid for 03 months)
- Copy of civil servant's certificate of employment (valid for 06 months)
- Release form signed by guardian

NB: The deadline for submitting claims for compensation is six(06) months.

XII- BRACES AND ODF

This assistance is granted to civil servants, their spouses or children, if they are not covered by CNAS. The reimbursement rate is 70% up to a maximum of 50,000DA.

XIII- OPHTHALMOLOGY AND EYEWEAR

All employees are eligible for financial assistance for eye care. The amount granted is 70% (with a ceiling of 50,000 DA).

The amount allocated for glasses is capped at 10,000 DA. It is granted once every two years.
*NB// minor children and spouses who are not dependent on the employee for CNAS purposes will not be eligible for any form of reimbursement.

XIV- ASSISTANCE WITH THE PURCHASE OF ORTHOPEDIC OR SPEECH-LANGUAGE AIDS

This assistance is granted to civil servants, their spouses or children, if they are not covered by CNAS. The reimbursement rate is 70% up to a maximum of 100,000DA.
