Avis de consultation n°05 /SMG/FM/UMMTO/2018Chapter 22-13 article 01The Faculty of Medicine at the Université Mouloud MAMMERI in Tizi-Ouzou is launching a tender for the acquisition of Stationery and office supplies  within the operating budget 2018 chapter 22-13, article 01.The specifications are structured in a single batch:Single lot: stationery and office supplies.The present consultation will be publicly posted and published on the website of the Université Mouloud MAMMERI de Tizi-Ouzou. www.ummto.dzQualified bidders are invited to obtain the specifications from the general resources department of the Faculty of Medicine, Université Mouloud MAMMERI, Tizi-Ouzou.Tenders must include a application forma technical proposal and a financial offer. Each offer is inserted in a closed and sealed envelope, indicating the reference and subject of the consultation as well as the words "application file" - "technical offer" - "financial offer". These three envelopes are placed in another anonymous envelope, bearing the words: "to be opened only by the bid opening and evaluation committee". specifying the number and subject of the consultation and addressed to:Dean of the Faculty of Medicine Consultation N° 05 /SMG/FM/UMMTO/ 2018Wearing "Stationery and office supplies"The file must include the following documents:A/ Application form :
  • A completed, signed, dated and sealed declaration of candidacy;
  • A signed, dated and sealed declaration of probity;
  • Copy of articles of association for companies ;
  • Copy of trade register ;
  • Copy of tax identification ;
  • Extract from the audited roll or with schedule dated less than one (1) month prior to the date of submission;
  • Original police record less than three months old at the time of submission;
  • Bank account number and address ;
  • Financial statements for the last three years, along with profit and loss statements (TCR).
B/ Technical offer :
  • List of specifications;
  • Declaration to be completed, signed, dated and sealed ;
  • Up-to-date certificate issued by CASNOS and CNAS (originals) ;
  • A CNAS declaration certifying the number of employees;
  • List of company personnel and their specializations;
  • Certificates of satisfactory performance for the last three years;
  • Copy of the establishment's status (showing number of beds and capacity).
C/ Financial offer
  • Letter of tender duly completed, signed, dated and sealed ;
  • Detailed estimate and quantity signed, dated and sealed ;
  • Unit price schedule signed, dated and sealed.
A delay of thirteen (13) bidders have a period of time in which to prepare and submit their bids from : June 07, 2018The deadline for submission of bids is  June 19, 2018 à 13h00.Tenders will be opened in the presence of any bidders who wish to attend, on the same day as the deadline for submission of tenders. June 19 2018 à 13h30 in the meeting room of the Faculty of Medicine.The bid validity period is equal to the bid preparation period plus three (03) months.
